OSHA Fines Contractor $65K After Falling Beam Kills Worker

Feb. 26, 2019

A joint venture between two contractors now faces $65,300 in penalties for multiple safety violations following a worker being fatally struck by a steel beam. The employee was working on a light rail tunnel project in San Francisco when he was struck, according to OSHA.

On August 10 of last year, the employee was operating a rail crane to push two flat rail cars into a tunnel, and struck an overhead steel beam with the boom. The beam fell 13 feet from its support brackets and fatally crushed an employee below.

Investigators discovered the crane operators had not been trained, as well as the workers around the crane.
