The required amount of cover will always be specified on the design drawings, but no time should the cover be less than the maximum size of aggregate plus 5mm. The first part is a visual inspection by the clerk of works or equivalent. Right side images shows correct way of placing of reinforcement.

For anything other than most smallest of concreting jobs e.g. walls over 1m in height, a drawing should be provided, showing what formwork is required. This need not be an intricate design drawing, a simple sketch is usually sufficient. Always inspect the formwork prior to pouring.
