The structural engineer sizes the concrete column; the architect places it in the space. There is a code on the structural drawing that can be tracked to the schedule and the dimensions are noted. They both are confident to be following well established building-documenting practices.

And I mean, everything, getting IFC drawings, as-builts, shop-drawings, or magical, know it all 3-7D (+) models. That wall will then (possibly by a structural engineer) chopped into pieces and the sizes determined. That sentence I just wrote, is a mouthful, but do read it again. So, imagine a detail.
