The answers can be determined but only after documented due diligence using Task Based Risk Assessment (past blogs) and assessing feasibility of hazard controls. Most safety professionals are familiar with the Hazard Control Hierarchy shown right. Many factors determine if the risk reduction measure is practicable.

I’m finding clients who have excellent lockout/tagout programs but who have not taken the necessary step of identifying tasks where LOTO is not feasible. During my career in engineering and safety, I have observed thousands of machine setups and other tasks where LOTO is simply not possible.

Typically, these machines are used for low-risk tasks performed by skilled workers. Most safety pros will react with, “You have to be kidding me.” Despite the very low risk, OSHA is indeed citing for the things noted above. Document your findings. If not, estimate weight and stability and document your findings.

FDRsafety continues to encounter situations where worker training is incomplete or lacking, especially in safety. I recently attended the 2 nd Annual Process Safety Summit in Washington DC on Oct 15-16, 2019. I do not like standard safety signs and warnings because they typically mean little to the worker.

Many, if not most safety courses, are designed for administrative issues of compliance. We use risk assessment and feasible risk reduction methodology that is part of the ANSI B11 family of general industry machine safety standards. Risk assessment is done with a clipboard and pencil on the factory floor.

What’s often missed and not understood is the in-between, where tasks are not minor servicing but LOTO is not feasible. In situations where LOTO is not feasible, an ALTERNATIVE METHOD is needed.
